Your initial appointment will consist of a consultation where we will explain your diagnosis and discuss treatment options for your oral problem. In some cases, surgery can be performed on the same day as the consultation. However, for those with a complex medical history or treatment plan, an evaluation and a second appointment may be required to provide treatment on another day.

Preparing for your consultation

To ensure a smooth and efficient consultation, please provide the following information at your appointment:

  • Insurance Information

If you have medical or dental insurance, bring the necessary completed forms. This will save time and help us process any claims.

  • Surgical Referral Slip and X-rays

If applicable, bring your surgical referral slip and any x-rays

  • Medications List

A current list of all medications you are taking.

IMPORTANT INFORMATION FOR MINORS: All patients under the age of 18 must be accompanied by a parent or guardian during the consultation visit.
